The home offers approximately 3,900 square feet of living space on a spacious 0.58-acre lot with large indoor and outdoor areas for resident comfort and relaxation.
The home includes five large private bedrooms designed to provide comfort, safety, privacy, and personalized care for residents.
Yes. Four of the five bedrooms include their own private bathroom area with wheelchair-accessible space, a sink, and convenient storage space, providing residents with added comfort, privacy, and independence in a warm home-like setting.
Yes. The home includes wide open living areas and a large fully accessible roll-in bathroom designed for wheelchairs, walkers, and Hoyer lift accessibility.
We support residents with a wide range of care needs including dementia care, hospice care, wheelchair support, mobility assistance, behavioral support, high-care medical conditions, and daily living assistance.
Our caregivers have between 5 and 16 years of hands-on caregiving experience and are trained to provide compassionate, respectful, and person-centered care.
Yes. Premier maintains at least one awake caregiver throughout the entire night to provide safety checks, supervision, and immediate assistance whenever needed.
The home maintains a minimum of two caregivers at all times, with additional staff available depending on resident needs.
Yes. The administrator and two caregivers reside on the property and remain available on-call 24 hours a day, 7 days a week.
Staff members are trained in caregiving, resident supervision, transfers, mobility assistance, medication administration, emergency response, documentation, and resident-centered care practices.
Yes. Premier Adult Family Home is licensed and regulated by the Oregon Department of Human Services and follows Oregon Administrative Rules OAR 411-049, OAR 411-050, OAR 411-051, and OAR 411-052 for Adult Foster Homes and residential care services.
We securely store, administer, document, and monitor all medications according to physician orders and Oregon state regulations.
Residents receive balanced home-cooked meals prepared daily using USDA MyPlate nutrition guidelines, physician orders, and personal dietary preferences.
Yes. We gladly accommodate diabetic diets, low-sodium diets, soft-food diets, texture-modified meals, and other physician-ordered dietary needs. We also work closely with residents and families to learn each resident’s personal food preferences, favorite meals, and dislikes so meals can feel enjoyable, comforting, and familiar while still supporting their health and nutritional needs.

Yes. The home has modern heating and air conditioning systems that are regularly serviced and maintained.
The home and landscaping are professionally maintained regularly to ensure a clean, safe, and beautiful environment year-round.
We take pride in maintaining a very clean and safe environment while following infection prevention practices to protect vulnerable residents.
We monitor infection risks carefully and may ask visitors or family members with contagious symptoms to wait until they are well before visiting to help protect resident health and safety.
The home maintains emergency food, bottled water, flashlights, batteries, first aid supplies, sanitation supplies, blankets, medical supplies, emergency communication equipment, and resident emergency go-bags.
Go-bags include hygiene supplies, clothing, medications, blankets, towels, important documents, comfort items, and personal necessities.
Yes. The home is equipped with a backup generator and fuel supply capable of supporting the home during extended power outages.
Yes. The home has emergency preparedness procedures and access to an emergency relocation residence if evacuation is ever needed.
The home maintains hospital beds, wheelchairs, Hoyer lifts, transfer equipment, eating trays, mobility devices, and additional care equipment for resident support.
Transportation arrangements can be coordinated for appointments, errands, and community needs as appropriate.
Each resident receives a personalized care plan based on their medical needs, mobility, medications, nutrition, routines, preferences, and physician recommendations.
Staff and administration maintain open and regular communication with families regarding resident care, health changes, appointments, concerns, and important updates. We communicate through phone calls, text messages, email, fax, and face-to-face conversations during family visits to help families stay informed, involved, and connected with their loved one’s care.

Yes. Families are welcome to bring personal belongings and decorations to help residents feel more comfortable and at home.
